Overview

Sivelestat is a small molecule inhibitor of human neutrophil elastase. It is primarily used for the treatment of acute lung injury and acute respiratory distress syndrome (ARDS), particularly in patients with systemic inflammatory response syndrome. Sivelestat works by selectively inhibiting neutrophil elastase, a serine protease involved in inflammation and tissue damage during acute lung injury. By blocking this enzyme, sivelestat reduces inflammatory responses and cytokine production (such as IL-8 and TNF-α), thereby mitigating lung injury without significantly impairing host immune defense. The drug has been approved for use in Japan and the Republic of Korea but remains investigational elsewhere. Preliminary studies also suggest potential benefits in neuropathic pain[1][3][4][5][6].
