Overview

SJYC07 High-Risk Therapy is a specific treatment arm within the SJYC07 clinical trial protocol developed by St. Jude Children's Research Hospital. This risk-adapted regimen is designed for young children (typically under 3 or 5 years old) newly diagnosed with high-risk central nervous system tumors, including medulloblastoma, pineoblastoma, and high-grade glioma. The therapy consists of three phases: induction, consolidation, and maintenance. Induction involves a combination of methotrexate, vincristine, cisplatin, cyclophosphamide, and vinblastine. Consolidation includes targeted intravenous topotecan and cyclophosphamide or craniospinal irradiation. Maintenance therapy utilizes oral cyclophosphamide, topotecan, and depending on the diagnosis, either erlotinib or etoposide. The protocol aims to improve survival while minimizing neurocognitive deficits by delaying or reducing radiation exposure in very young patients.
