SkQ1 is a first-in-class, mitochondria-targeted small molecule antioxidant developed by Mitotech for the treatment of age-related disorders, primarily dry eye disease (DED). The molecule consists of a plastoquinone moiety linked to a lipophilic triphenylphosphonium cation, which enables it to selectively accumulate in the inner mitochondrial membrane. Its mechanism of action involves neutralizing mitochondrial reactive oxygen species (ROS) and inhibiting cardiolipin peroxidation, thereby addressing the oxidative stress and inflammation central to DED pathogenesis. SkQ1 is a "renewable" antioxidant, as it can be re-reduced by the mitochondrial electron transport chain after being oxidized, providing sustained protection. It is approved in Russia under the brand name Visomitin and has completed Phase 3 clinical trials (VISTA-1 and VISTA-2) in the United States, demonstrating efficacy in reducing corneal staining and improving visual function.
