It is a **small-molecule beta-lactam antibiotic** intended for treatment of **community-acquired bacterial infections**, with reported potent in vitro and in vivo activity against pathogens including *Streptococcus pneumoniae* including penicillin-resistant strains, *Haemophilus influenzae* including beta-lactamase-negative ampicillin-resistant strains, *Staphylococcus aureus* susceptible strains, *Streptococcus pyogenes*, *Enterococcus faecalis*, *Klebsiella pneumoniae*, *Moraxella catarrhalis*, and *Neisseria gonorrhoeae*. As a carbapenem, its therapeutic effect is understood to result from **binding to bacterial penicillin-binding proteins and inhibition of peptidoglycan cell-wall synthesis**, producing bactericidal activity. Published preclinical data describe **intravenous** activity and pharmacokinetics broadly similar to meropenem in monkeys, stability against human dehydropeptidase I, and note that the medoxomil ester prodrug **SM-375769** was explored for oral delivery. No evidence of an approved marketed product name for SM-369926 itself was identified.
