Overview

SM04646 is a small molecule inhibitor of the Wnt signaling pathway developed for the treatment of idiopathic pulmonary fibrosis (IPF). It acts as an antifibrotic agent by inhibiting Wnt/beta-catenin signaling, which regulates cell growth and differentiation and is implicated in fibrotic diseases. Preclinical studies demonstrated that SM04646 attenuates fibrosis by inhibiting expression of genes stimulated by transforming growth factor-beta (TGF-beta), such as Frizzled-8 (FZD-8) and WISP-1. The drug was delivered as an inhaled aerosol and showed greater anti-fibrotic activity in vitro compared to existing therapies like pirfenidone or nintedanib. Clinical development included phase 1 trials showing safety and tolerability in healthy volunteers; however, further development for IPF was discontinued after phase 2[1][2][4][5][6].
