SMC1A frameshift peptide vaccine is a therapeutic cancer vaccine targeting neoantigens resulting from frameshift mutations in the Structural Maintenance of Chromosomes 1A (SMC1A) gene. These mutations are frequently observed in tumors characterized by microsatellite instability (MSI), such as colorectal, gastric, and endometrial cancers. The vaccine typically consists of synthetic long peptides (SLPs) that encompass the novel amino acid sequences generated by the frameshift mutation (e.g., c.1603_1604delAT). Its mechanism of action involves the uptake of these peptides by antigen-presenting cells, which then present the neoantigens to CD8+ and CD4+ T cells, eliciting a robust and specific anti-tumor immune response. While often investigated as a component of multi-peptide "off-the-shelf" vaccine panels designed for MSI-high (MSI-H) or mismatch repair-deficient (dMMR) patients, the SMC1A-specific approach focuses on one of the most frequent and immunogenic shared neoantigens in the MSI landscape.
