Smilax glabra total glycosides (SGTG) is a botanical drug candidate derived from the rhizome of *Smilax glabra* Roxb., a plant widely used in traditional Chinese medicine (Tu Fu Ling). Developed by researchers at Nanjing University, SGTG is primarily investigated for its potent immunomodulatory and anti-inflammatory properties in the treatment of rheumatoid arthritis (RA). The extract's therapeutic efficacy is largely attributed to its major active constituent, the flavonoid glycoside astilbin. SGTG acts by selectively inhibiting the over-activation and proliferation of T-lymphocytes and suppressing the production of pro-inflammatory cytokines, including tumor necrosis factor-alpha (TNF-α), interleukin-1 beta (IL-1β), and interleukin-6 (IL-6). Its molecular mechanism involves the modulation of the NF-κB signaling pathway and the induction of apoptosis in activated T cells, thereby reducing joint inflammation and preventing bone destruction without the systemic toxicity often associated with conventional immunosuppressants.
