**SmofKabiven extra Nitrogen** is a three-chamber bag emulsion for intravenous parenteral nutrition, containing a lipid mixture (soybean oil, medium-chain triglycerides, olive oil, fish oil), glucose, and a high concentration of amino acids designed for patients with elevated nitrogen needs. It provides a balanced supply of energy (from lipids and glucose), proteins (amino acids delivering up to 0.28 g nitrogen/kg/day at maximum dose), and essential fatty acids including omega-3s, without electrolytes in some formulations, for use when oral or enteral nutrition is impossible, insufficient, or contraindicated. Administered via central vein over 12-24 hours, it supports adults, adolescents (12-18 years), children ≥2 years, and geriatrics, with dosing tailored to body weight, metabolic stress, and clinical condition (max 31-35 ml/kg/day). Developed and manufactured by Fresenius Kabi, it improves fatty acid profiles compared to standard lipid emulsions in pediatric studies and is evaluated in critical illness for achieving protein targets[1][2][7][12][15].
