Sodium arsenite (sodium metaarsenite) is a trivalent inorganic arsenic compound that acts as a small molecule telomerase inhibitor with antineoplastic activity. It primarily targets telomeric DNA sequences (specifically TTAGGG repeats), leading to telomere shortening and the translocation of the catalytic subunit of telomerase (hTERT) from the nucleus to the cytoplasm, which induces apoptosis and inhibits tumor cell proliferation. Beyond telomerase inhibition, sodium arsenite modulates several intracellular signaling pathways, including the suppression of the IKK-NF-kappaB and JAK2-STAT3 pathways and the activation of the MAPK/JNK-cJun pathways. Developed by Komipharm International under the code KML-001, it has been investigated in clinical trials for advanced solid tumors, prostate cancer, and acute promyelocytic leukemia. Historically, it has also been utilized as an antibacterial agent, antifungal agent, herbicide, and insecticide.
