SAGM (Sodium-Adenine-Glucose-Mannitol) is a specialized additive solution used in transfusion medicine to extend the shelf life of red blood cells (RBCs) after they have been separated from whole blood. By replacing the plasma removed during processing, SAGM maintains RBC viability and functionality for up to 42 days under refrigeration (4-6 °C). The formulation consists of sodium chloride to maintain isotonicity, adenine to facilitate the maintenance of high-energy phosphate (ATP) levels, glucose to provide a substrate for anaerobic metabolism, and mannitol to act as an osmotic stabilizer that protects the red cell membrane against hemolysis. This preservative solution is essential for blood banking operations, ensuring a stable supply of blood products for patients requiring transfusions due to surgery, trauma, or chronic anemia.
