Overview
**Sodium citrate + ethanol** is a sterile, single-use catheter-lock solution containing 4% sodium citrate and 30% ethanol, marketed as **CitraFlow PLUS** and regionally as **CitraLok PLUS**. It is instilled into the lumen of an indwelling vascular-access or hemodialysis catheter between treatments rather than administered as a systemic infusion. Citrate chelates ionized calcium to inhibit calcium-dependent coagulation within the catheter, helping maintain patency and prevent thrombosis, while ethanol provides broad local antimicrobial activity and may reduce intraluminal biofilm formation. A University of Manitoba randomized pilot study evaluated the 30% ethanol/4% sodium citrate lock versus heparin for prevention of catheter-related infection, bacteremia, and thrombosis in patients with stage V chronic kidney disease receiving hemodialysis.
