Overview
Sodium iodide I-124 is a positron-emitting radiopharmaceutical used as a diagnostic imaging agent for PET/CT. It contains the radioisotope iodine-124, which is actively transported into thyroid cells via the sodium iodide symporter (NIS). Its primary clinical application is for pre-therapeutic individualized lesion dosimetry in patients with differentiated thyroid carcinoma (DTC), particularly those with lung or bone metastases, to optimize subsequent treatment with iodine-131. By providing high-resolution PET images, it allows for more accurate quantification of iodine uptake and residence time compared to traditional planar imaging or SPECT. It is currently being evaluated in clinical trials as an investigational tool for personalized dosimetry.
