Sodium oligomannate is a mixture of linear, acidic oligosaccharides derived from the marine algae Ecklonia kurome. It is used in China for the treatment of mild to moderate Alzheimer's disease to improve cognitive function. The drug was conditionally approved in China in 2019 but has not been approved elsewhere due to insufficient clinical data. Its mechanism of action is not fully understood, but proposed mechanisms include inhibition and destabilization of amyloid beta aggregation, modulation of gut microbiota leading to reduced neuroinflammation, and prevention of inflammatory responses associated with amyloid plaques. Sodium oligomannate modestly penetrates the blood-brain barrier and may act through multiple pathways related to both amyloid pathology and inflammation[1][3][7][8].
