Sodium phosphate, dibasic, anhydrous and sodium phosphate, monobasic, monohydrate is a combination of inorganic salts used as an osmotic laxative and for electrolyte replacement. In its oral tablet form (e.g., OsmoPrep, Visicol), it functions by increasing the osmotic pressure within the intestinal lumen, which draws water into the colon, softens the stool, and stimulates bowel evacuation. This is primarily indicated for cleansing the colon in preparation for colonoscopy or surgery in adults. The drug is also available as an intravenous injection used to treat or prevent hypophosphatemia and as a source of phosphate in parenteral nutrition. A significant safety concern associated with the oral formulation is the risk of acute phosphate nephropathy, which can lead to permanent renal impairment, particularly in patients with pre-existing kidney disease or those taking certain medications like ACE inhibitors or NSAIDs.
