This is a combination of three repurposed drugs—sodium valproate (also known as valproic acid), bezafibrate, and medroxyprogesterone acetate—investigated primarily for its anti-cancer properties. The combination, sometimes referred to as "V-BAP," has demonstrated selective inhibition of growth and survival in human osteosarcoma cell lines without adversely affecting normal mesenchymal stem/stromal cells. Mechanistically, the combination alters lipid metabolism in cancer cells by reducing levels of fatty acid synthase and stearoyl CoA desaturase 1 prior to cell death. Separately, combinations of bezafibrate and medroxyprogesterone acetate have shown anti-leukemic activity through increased synthesis and reduced metabolism of prostaglandin D2, leading to elevated levels of the anti-neoplastic prostaglandin 15-deoxy-Δ12,14-PGJ2. Sodium valproate is a histone deacetylase inhibitor with antiepileptic properties; bezafibrate is a pan-PPAR agonist used for dyslipidemia; medroxyprogesterone acetate is a synthetic progestin used in hormonal therapies[1][2][3].
