Overview

Sofpironium is a **topical anticholinergic small molecule** developed by Botanix and previously by Brickell Biotech for the treatment of **primary axillary hyperhidrosis**. Its active moiety is sofpironium bromide, formulated as a topical gel, and it is marketed as **Sofdra** in the United States and **Ecclock** in Japan. Sofpironium is a retrometabolically designed anticholinergic that inhibits **muscarinic acetylcholine receptors, particularly the M3 receptor on eccrine sweat glands**, thereby reducing sweat production in the axillae. The compound undergoes rapid nonenzymatic hydrolysis, which is intended to limit systemic exposure and reduce anticholinergic adverse effects while maintaining local efficacy.
