Soluble protein idiotype vaccine is a patient-specific immunotherapy designed primarily for B-cell lymphomas. The idiotype, defined as the collection of idiotopes (epitopes found on the hypervariable regions of an immunoglobulin), serves as a unique tumor-specific antigen for each patient's B-cell malignancy. The vaccine is typically produced by isolating the patient's tumor-specific idiotype protein using rescue hybridoma methods, then conjugating it with keyhole limpet hemocyanin (KLH) to enhance its immunogenicity. Granulocyte-monocyte colony-stimulating factor (GM-CSF) is commonly added as an immunologic adjuvant. The vaccine aims to elicit idiotype-specific immune responses capable of eliminating tumor cells. Clinical trials have shown both biological and clinical efficacy, particularly in follicular lymphoma patients, though several phase III trials have failed to achieve their primary endpoints.
