Sonolisib (PX-866) is an oral, irreversible, small molecule inhibitor of phosphoinositide 3-kinase (PI3K), specifically targeting the alpha, gamma, and delta isoforms. It is a synthetic analogue of wortmannin with improved stability and reduced toxicity compared to its parent compound. Sonolisib inhibits the production of the secondary messenger phosphatidylinositol-3,4,5-trisphosphate (PIP3), thereby blocking activation of the PI3K/Akt/mTOR signaling pathway. This inhibition results in decreased tumor cell proliferation and survival by suppressing downstream effectors such as Akt and mTOR. The drug has demonstrated antitumor activity in preclinical models and has been evaluated in clinical trials for advanced solid tumors.
