This investigational combination therapy consists of the multi-kinase inhibitor sorafenib and the traditional Chinese medicine formulation Realgar-Indigo Naturalis Formula (RIF), also known as Compound Huangdai Tablets. RIF is a four-ingredient mixture containing realgar (tetraarsenic tetrasulfide), Indigo naturalis (indirubin), Salvia miltiorrhiza (tanshinone IIA), and Pseudostellaria heterophylla. While RIF is an approved treatment in China for acute promyelocytic leukemia (APL), where it targets the PML-RARα fusion protein, this combination is being evaluated by Zhejiang University for the treatment of recurrent hepatocellular carcinoma (HCC) following liver transplantation. The mechanism of action involves sorafenib's inhibition of tumor angiogenesis and cell proliferation via VEGFR, PDGFR, and Raf kinase pathways, potentially complemented by the anti-tumor and synergistic effects of RIF's active components.
