Sorbinil is a small molecule aldose reductase inhibitor developed for the treatment of diabetic complications such as neuropathy and retinopathy. Aldose reductase is an enzyme involved in the polyol pathway, converting excess glucose to sorbitol during hyperglycemia. Accumulation of sorbitol can contribute to diabetic complications including cataracts and peripheral neuropathy. Sorbinil inhibits aldose reductase by binding to the enzyme complexed with oxidized nicotinamide dinucleotide phosphate (NADP+), forming a dead-end complex that prevents enzymatic turnover and reduces flux through the polyol pathway[1][2][6]. Preclinical studies demonstrated efficacy in reducing sorbitol accumulation in animal models, and clinical trials investigated its use for painful diabetic neuropathy[4][5]. However, advanced clinical development was not completed.
