Overview

**sorivudine + valaciclovir** is a theoretical combination of two oral antiviral drugs, both belonging to the nucleoside analog class, primarily investigated or used individually for the treatment of herpesvirus infections, particularly herpes zoster (shingles). Sorivudine is a thymidine analog with potent activity against varicella-zoster virus (VZV), inhibiting viral DNA replication by targeting viral DNA polymerase after monophosphorylation by the viral thymidine kinase; it is notable for high oral bioavailability. Valaciclovir is the L-valine ester prodrug of aciclovir and is rapidly converted in the body to aciclovir, a guanine nucleoside analog that requires initial phosphorylation by viral thymidine kinase. The resultant aciclovir triphosphate inhibits viral DNA polymerase through competitive inhibition, viral DNA chain termination, and inactivation of viral polymerase. Both compounds target DNA viruses, specifically herpes simplex viruses (HSV-1, HSV-2), VZV, and with variable effect against others like Epstein-Barr virus (EBV). Each has been developed and approved as monotherapy, but no evidence supports approval, routine use, or clinical development of their fixed combination; such co-use could pose increased risk of drug interactions or overlapping toxicities, especially given sorivudine's known lethal interaction with 5-fluorouracil and closely related anti-cancer drugs[1][2][6].
