Spanlecortemlocel (formerly MGTA-456) is an ex vivo expanded hematopoietic stem cell (HSC) therapy designed to improve outcomes for patients undergoing umbilical cord blood (UCB) transplantation. The product consists of UCB-derived CD34+ cells that have been expanded using **StemRegenin-1 (SR-1)**, a small molecule **aryl hydrocarbon receptor (AHR) antagonist**, which promotes the proliferation of hematopoietic stem and progenitor cells while inhibiting their differentiation. This expansion process aims to overcome the cell dose limitations typically associated with UCB, leading to faster neutrophil and platelet engraftment and reduced transplant-related complications. Originally developed by **Novartis** as HSC835, the program was licensed to **Magenta Therapeutics** in 2017 for development in inherited metabolic disorders (IMD) and high-risk hematological malignancies. However, Magenta Therapeutics terminated the license agreement and discontinued the development of MGTA-456 in late 2021.
