Overview

Spesolimab is a first-in-class humanized monoclonal antibody that blocks the interleukin-36 receptor, thereby preventing IL-36 ligand signaling and downstream pro-inflammatory immune activation. It was originated by Boehringer Ingelheim and is marketed as Spevigo for generalized pustular psoriasis, where it has shown rapid activity in acute flares and has also been studied for flare prevention. The asset was later licensed globally to LEO Pharma for commercialization and further development in dermatology. Beyond generalized pustular psoriasis, spesolimab has been investigated in palmoplantar pustulosis, atopic dermatitis, hidradenitis suppurativa, and Crohn's disease, including perianal fistulizing and fibrostenotic disease.
