Overview

This combination consists of **spevatamig** (also known as PT886), a first-in-class native IgG-like bispecific antibody targeting claudin 18.2 and CD47, and **pembrolizumab** (Keytruda), a monoclonal antibody targeting PD-1 (programmed death receptor-1). - **Spevatamig** is designed to both block the immunosuppressive CD47 signal (a "don't eat me" signal to macrophages) and engage claudin 18.2, a tight junction protein overexpressed in certain cancers, aiming to potentiate anti-tumor immune activity. - **Pembrolizumab** is a humanized IgG4 monoclonal antibody that blocks PD-1 on T-cells, removing inhibition of immune activation and increasing anti-tumor effects. The combination is under clinical investigation for the treatment of advanced solid tumors, particularly biliary tract carcinoma, pancreatic cancer, gastric, and gastroesophageal junction adenocarcinomas, with focus on tumors expressing claudin 18.2 or CD47. The combination is not currently approved and is being evaluated in ongoing Phase I/II studies[5][7][9].
