**SPI-47** is Sparrow Pharmaceuticals' investigational fixed-dose oral combination of **clofutriben** and **prednisolone** being developed for polymyalgia rheumatica. Clofutriben is a potent, selective inhibitor of corticosteroid 11-beta-dehydrogenase isozyme 1, an intracellular enzyme that regenerates active cortisol from cortisone in tissues associated with glucocorticoid toxicity. Prednisolone is a glucocorticoid receptor agonist that suppresses inflammation. The combination is intended to preserve prednisolone's anti-inflammatory benefit while reducing tissue glucocorticoid exposure and steroid-related toxicity.
