Overview

Spirapril is an oral, long-acting angiotensin-converting enzyme (ACE) inhibitor used primarily for the treatment of hypertension. It belongs to the dicarboxy group of ACE inhibitors and acts as a prodrug, being converted in vivo to its active metabolite, spiraprilat. Spiraprilat competitively inhibits ACE, blocking the conversion of angiotensin I to angiotensin II—a potent vasoconstrictor—and thereby reduces blood pressure by promoting vasodilation and decreasing aldosterone secretion. This leads to reduced sodium reabsorption and increased water excretion. Unlike many other ACE inhibitors, spirapril is eliminated via both renal and hepatic routes, which may be advantageous in patients with renal impairment. In addition to hypertension, it has been studied for use in congestive heart failure and type II diabetes[1][2][5][6].
