Spironolactone is a synthetic steroidal antimineralocorticoid and potassium-sparing diuretic. It acts primarily as a competitive antagonist of the mineralocorticoid receptor, thereby inhibiting the effects of aldosterone in the distal renal tubules and collecting ducts. This leads to increased excretion of sodium and water while conserving potassium. Spironolactone also exhibits antiandrogenic activity by antagonizing androgen receptors and weak progestogenic/estrogenic effects due to its interaction with other steroid hormone receptors. Clinically, it is used for treating heart failure (especially NYHA class III-IV), hypertension (as add-on therapy), edema associated with liver cirrhosis or nephrotic syndrome, primary hyperaldosteronism (diagnosis and treatment), hypokalemia prevention/treatment, hirsutism in women, adult acne vulgaris, female pattern hair loss, polycystic ovary syndrome-related symptoms such as excess hair growth, and as part of feminizing hormone therapy for transgender women[1][2][4][5][6][7][9].
