Spirulina-derived exosome hydrogel is an investigational biologic therapy developed by West China Hospital for the prevention and treatment of radiation-induced esophagitis (RIE) in patients with esophageal cancer. The product consists of purified exosomes—extracellular vesicles—derived from the cyanobacterium *Arthrospira platensis* (spirulina), which are formulated into an oral hydrogel. These plant-like exosomes are thought to possess potent anti-inflammatory and antioxidant properties, which help to mitigate the mucosal damage and oxidative stress caused by intensity-modulated radiotherapy (IMRT). The hydrogel formulation is designed to provide a protective coating on the esophageal lining, facilitating localized delivery and prolonged contact of the therapeutic vesicles with the affected tissue.
