SPN-830 (brand name Onapgo) is a wearable subcutaneous infusion device that delivers continuous apomorphine for the treatment of motor fluctuations ("off" episodes) in adults with advanced Parkinson's disease. Apomorphine is a non-ergoline dopamine agonist that stimulates postsynaptic dopamine D2-type receptors within the caudate-putamen in the brain, helping to reduce "off" time and improve motor control. The device provides a continuous subcutaneous infusion of apomorphine throughout waking hours via an electronic pump typically placed on the abdomen or upper thigh. Clinical trials (TOLEDO and INFUS-ON) demonstrated significant reductions in daily "off" time and increases in "on" time without troublesome dyskinesia compared to placebo. The most common side effects include infusion site reactions, nausea, somnolence, headache, insomnia, and dyskinesia[6][7][8][9].
