**SPR741 + piperacillin + tazobactam** is an investigational antimicrobial combination comprised of SPR741, a synthetic polymyxin B analog, and piperacillin-tazobactam, a broad-spectrum beta-lactam/beta-lactamase inhibitor antibiotic. SPR741 acts by interacting with and disrupting the outer membrane of Gram-negative bacteria, thereby enhancing the permeability and uptake of coadministered antibiotics. While SPR741 has minimal intrinsic antibacterial activity, its addition potentiates the efficacy of antibiotics like piperacillin-tazobactam against multidrug-resistant Gram-negative organisms. Piperacillin inhibits bacterial cell wall synthesis, and tazobactam inhibits beta-lactamase enzymes, protecting piperacillin from enzymatic degradation. The combination is in clinical development for the treatment of serious infections caused by drug-resistant Gram-negative pathogens, including urinary tract infections. Safety and pharmacokinetic studies show good tolerability of SPR741 alone and in combination; no significant changes in drug clearance or adverse pharmacokinetic interactions have been reported when SPR741 is administered with piperacillin-tazobactam[1][3][2][5].
