ST-04

ST-04 is a mucoadhesive intraurethral polymer suspension of alprostadil (prostaglandin E1) currently in Phase 2 clinical development for the treatment of erectile dysfunction (ED). Developed by Dr. Ryan Flannigan at the University of British Columbia, the formulation is designed to be applied intraurethrally, where it forms a hydrogel that adheres to the mucosal lining of the urethra. This delivery mechanism is intended to facilitate the localized absorption of alprostadil into the surrounding tissues, including the corpora cavernosa. Alprostadil acts as a vasodilator by binding to prostaglandin E (EP) receptors, which increases intracellular cyclic adenosine monophosphate (cAMP) levels and leads to the relaxation of trabecular smooth muscle and dilation of cavernosal arteries. This process promotes penile tumescence and erection. ST-04 is being investigated for its efficacy both as a monotherapy and in potential combination with sildenafil, particularly for patients who may not respond adequately to oral phosphodiesterase-5 (PDE5) inhibitors.
