Overview

Stable isotope labeled levothyroxine refers to synthetic levothyroxine (L-thyroxine, T4) in which one or more atoms have been replaced with non-radioactive heavy isotopes such as carbon-13 (^13C) or deuterium (^2H/d3)[1][2][10]. These compounds are chemically and pharmacologically identical to regular levothyroxine but can be distinguished from endogenous hormone using mass spectrometry. They are primarily used as tracers in pharmacokinetic studies to precisely measure absorption, distribution, metabolism, and elimination of exogenous T4 without interference from the body’s own hormone[1][3]. The mechanism of action is identical to standard levothyroxine—acting as a replacement for deficient thyroxine by binding thyroid hormone receptors and regulating gene expression involved in metabolic processes.
