The "standard anti-TB regimen" refers to the internationally recommended first-line combination therapy for drug-susceptible active tuberculosis (TB). This regimen consists of an initial intensive phase with four drugs—isoniazid, rifampin (also known as rifampicin), pyrazinamide, and ethambutol—administered for two months. This is followed by a continuation phase with isoniazid and rifampin for four additional months. The mechanism of action involves inhibition of mycolic acid synthesis (isoniazid), inhibition of DNA-dependent RNA polymerase (rifampin), disruption of mycobacterial cell membrane metabolism and transport functions (pyrazinamide), and inhibition of arabinosyl transferases involved in cell wall biosynthesis (ethambutol). The primary indication is the treatment of active drug-susceptible pulmonary TB in adults and adolescents[1][4][7][9].
