The standard lymphodepletion regimen used in the Stanford University Phase 1 trial for multiple sclerosis refers to the preparatory chemotherapy protocol administered prior to the infusion of KYV-101, an autologous anti-CD19 CAR T-cell therapy. In this specific clinical context, the regimen utilizes bendamustine, a bifunctional alkylating agent with both nitrogen mustard and purine-like properties. The primary objective of this regimen is to deplete endogenous T-cells and B-cells in patients with non-relapsing and progressive forms of multiple sclerosis (including primary progressive and secondary progressive MS). By clearing the host's existing immune cells, the regimen creates a favorable environment for the engraftment, expansion, and persistence of the subsequent CAR T-cell therapy. Bendamustine acts by inducing intra-strand and inter-strand DNA cross-links, which leads to cell cycle arrest and apoptosis in rapidly dividing cells.
