Standard Prophylaxis-The First Affiliated Hospital of Soochow University-acute graft-versus-host disease refers to a specific multi-drug immunosuppressive regimen used as the control arm in clinical trials (such as NCT05149365) for the prevention of acute graft-versus-host disease (aGVHD). This regimen is administered to patients undergoing alternative donor allogeneic hematopoietic stem cell transplantation (HSCT). It typically consists of four components: antithymocyte immunoglobulin (ATG), cyclosporine A (CsA), mycophenolate mofetil (MMF), and methotrexate (MTX). Each component targets a different aspect of the immune response: ATG provides T-cell depletion; cyclosporine is a calcineurin inhibitor that prevents T-cell activation; mycophenolate mofetil inhibits inosine monophosphate dehydrogenase to block lymphocyte proliferation; and methotrexate acts as an antimetabolite by inhibiting dihydrofolate reductase. This combination is designed to provide comprehensive immunosuppression to prevent the donor's immune cells from attacking the recipient's tissues. In some clinical contexts, tacrolimus may be substituted for cyclosporine within this standard framework.
