This intensified four-drug regimen is being evaluated for the treatment of disseminated Mycobacterium avium complex (MAC) infection, particularly in patients with advanced HIV. The regimen consists of the standard triple therapy—comprising a macrolide (such as clarithromycin or azithromycin), ethambutol, and a rifamycin (typically rifabutin)—supplemented with a fluoroquinolone (either levofloxacin or moxifloxacin). The addition of the fluoroquinolone aims to improve early clinical outcomes and microbiological clearance in patients with high mycobacterial burden or profound immunodeficiency, where standard therapy may be suboptimal. The regimen is currently being studied in a multicenter, randomized, open-label trial led by the Shanghai Public Health Clinical Center.
