Overview

Staphylococcal protein A-silica is an extracorporeal immunoadsorption device in which staphylococcal protein A (SPA), a cell wall component of Staphylococcus aureus with high affinity for the Fc region of human IgG antibodies, is covalently bound to a silica matrix. This device (not a traditional drug) is used to remove pathogenic autoantibodies from plasma by binding them as blood passes through the column. The primary mechanism involves selective adsorption of IgG and immune complexes via SPA’s interaction with the Fc region, thereby reducing circulating autoantibody levels. It has been FDA-approved for use in rheumatoid arthritis and idiopathic thrombocytopenic purpura (ITP), and has also been explored off-label for other autoimmune diseases such as glomerulonephritis, systemic lupus erythematosus, myasthenia gravis, Guillain-Barré syndrome, and alloantibody-mediated conditions related to transplantation[1]. The best-known commercial product using this technology was marketed under the name Prosorba.
