START-001 (also known as STAR0602) is a first-in-class, bispecific antibody-fusion protein developed by Marengo Therapeutics using its proprietary Selective T Cell Activation and Rejuvenation (STAR) platform. It is designed to selectively target and activate a specific subset of T cells by binding to the Vβ6 and Vβ10 regions of the T cell receptor (TCR). The molecule consists of a monoclonal antibody targeting these specific Vβ chains fused to a costimulatory 4-1BB ligand (4-1BBL) domain. This dual-signal mechanism provides both primary activation (Signal 1) and costimulation (Signal 2) in a Vβ-selective manner, promoting the expansion and rejuvenation of a "fit" T cell repertoire to enhance anti-tumor immunity. START-001 is currently being evaluated in Phase 1/2 clinical trials for the treatment of advanced solid tumors, including those that are refractory to standard-of-care checkpoint inhibitors.
