Stiripentol + carbamazepine is an investigational combination of two antiepileptic drugs, each with distinct mechanisms of action, explored primarily for polytherapy in refractory epilepsy. Stiripentol acts as an anticonvulsant by positively modulating gamma-aminobutyric acid type A (GABAA) receptors in the brain (positive allosteric modulator), enhancing inhibitory neurotransmission, and also inhibits cytochrome P450 enzymes, thus increasing plasma concentrations of co-administered anticonvulsants such as carbamazepine. Carbamazepine is a sodium channel blocker that stabilizes hyperexcited nerve membranes, inhibits repetitive neuronal discharges, and reduces synaptic propagation of excitatory signals. The combination is being studied for potential synergistic effects, with evidence suggesting stiripentol may suppress the formation of carbamazepine-epoxide (an active metabolite associated with toxicity) and allow dose reductions of carbamazepine while improving seizure control, especially in pharmacoresistant epilepsies[1][3][5][6].
