STK-038 is an engineered interleukin-22 (IL-22) partial agonist and surrogate cytokine agonist developed by Synthekine for the treatment of obesity and inflammatory diseases. It is designed to selectively modulate IL-22 signaling by having attenuated binding to the IL-10Rβ subunit of the heterodimeric IL-22Rα:IL-10Rβ receptor complex. This selective binding allows STK-038 to preferentially signal in colon epithelial cells while avoiding activation in skin keratinocytes, thereby decoupling the therapeutic metabolic benefits of IL-22 from dose-limiting dermatological toxicities (such as skin hyperplasia and inflammation). In preclinical models of diet-induced obesity, STK-038 has demonstrated efficacy in reducing body weight and elevating satiety hormones (such as GLP-1 and PYY), both as a monotherapy and in combination with GLP-1 receptor agonists like semaglutide.
