Stromal vascular fraction + functional self-assembling peptide nanofiber hydrogel is an investigational combination biologic and biomaterial therapy currently in Phase 1 clinical development for the treatment of spinal cord injury (SCI). The therapy consists of a stromal vascular fraction (SVF)—a heterogeneous population of adipose-derived cells including mesenchymal stem cells, endothelial progenitors, and immune cells—encapsulated within a synthetic self-assembling peptide (SAP) hydrogel. The hydrogel component is based on the RADA16-I peptide sequence functionalized with bioactive motifs: an HGF-mimetic peptide to promote cell survival and a laminin-derived RIKVAV sequence to facilitate neurite outgrowth and adhesion. This composite system is designed to be implanted into the spinal cord lesion site to provide structural support, reduce inflammation, and stimulate neuroregeneration and angiogenesis through both direct cell replacement and paracrine signaling. The primary clinical study is sponsored by Sun Yat-sen University (NCT05967325).
