STW 5-II is a multi-target herbal phytomedicine developed for the treatment of functional gastrointestinal disorders, specifically functional dyspepsia (FD) and irritable bowel syndrome (IBS). It is a refined six-herb formulation derived from the original nine-herb product STW 5 (Iberogast). The preparation consists of hydroethanolic extracts of bitter candytuft (*Iberis amara*), peppermint leaf (*Mentha x piperita*), chamomile flower (*Matricaria recutita*), licorice root (*Glycyrrhiza glabra*), caraway fruit (*Carum carvi*), and melissa leaf (*Melissa officinalis*). STW 5-II exerts its therapeutic effects through a multi-target approach: it modulates gastrointestinal motility in a region-specific manner (relaxing the proximal stomach while increasing distal contractility), reduces visceral hypersensitivity by desensitizing intestinal afferents, exhibits anti-inflammatory properties, and protects gut barrier integrity by stabilizing tight junction proteins. It has recently been introduced into various European markets for the management of disorders of gut-brain interaction (DGBIs).
