Overview

**Substance P-Saporin** is an investigational targeted neurotoxin conjugate developed for durable treatment of severe chronic pain, including opioid-refractory cancer pain. It consists of the endogenous tachykinin peptide substance P covalently linked through a disulfide-containing linkage to saporin, a type I ribosome-inactivating protein. Substance P directs the conjugate to neurokinin-1 receptor-expressing spinal neurons; receptor-mediated internalization enables saporin to enter the cytoplasm, inactivate ribosomes, stop protein synthesis, and permanently ablate these nociceptive relay neurons. It has been evaluated by intrathecal administration in a Phase 1 study in patients with terminal cancer and intractable pain.
