Sucroferric oxyhydroxide is an iron-based, non-calcium phosphate binder used to control serum phosphorus levels in patients with chronic kidney disease (CKD) who are on dialysis. Its mechanism of action involves binding dietary phosphate in the gastrointestinal tract through ligand exchange between hydroxyl groups and/or water in sucroferric oxyhydroxide and dietary phosphate, forming insoluble complexes that are excreted in the feces. This reduces both serum phosphorus and calcium-phosphorus product levels by decreasing dietary phosphate absorption. Sucroferric oxyhydroxide is minimally absorbed systemically, reducing the risk of iron overload compared to other iron-based medications. It offers a lower pill burden than some alternative binders and does not contribute to vascular calcification, unlike calcium-based binders[1][2][3][4][5][6].
