**Sucroferric oxyhydroxide (PA21)** is an iron-based, non-calcium phosphate binder used to control serum phosphorus levels in patients with chronic kidney disease (CKD) on dialysis. It acts by binding dietary phosphate in the gastrointestinal tract and preventing its absorption[6][8]. The active component is a complex of iron(III) oxyhydroxide with sucrose and starches. Developed as an alternative to calcium-based binders, it helps reduce hyperphosphatemia and associated complications such as renal osteodystrophy[6][8]. **Losartan** is an angiotensin II receptor blocker (ARB) used primarily for treating hypertension (high blood pressure), reducing the risk of stroke in patients with left ventricular hypertrophy, and managing diabetic nephropathy in type 2 diabetes patients. Losartan works by blocking angiotensin II from binding to its receptor on blood vessels, leading to vasodilation and reduced blood pressure[2][3][5]. It also offers renal protection in diabetic kidney disease. This combination would be considered when both phosphate control (in CKD/dialysis) and hypertension or cardiovascular/renal protection are needed.
