Sugemalimab + olaparib + carboplatin + etoposide is an investigational combination therapy regimen being evaluated for the first-line treatment of extensive-stage small cell lung cancer (ES-SCLC). This regimen integrates four distinct therapeutic approaches: immune checkpoint inhibition via sugemalimab (an anti-PD-L1 monoclonal antibody), DNA damage repair inhibition via olaparib (a PARP1/2 inhibitor), and cytotoxic chemotherapy using the standard-of-care doublet of carboplatin (a platinum-based alkylating agent) and etoposide (a topoisomerase II inhibitor). The combination aims to exploit the high mutational burden and DNA repair deficiencies often found in SCLC. By combining these agents, the regimen seeks to enhance anti-tumor activity through synergistic mechanisms, where chemotherapy-induced DNA damage is further exacerbated by PARP inhibition, and the resulting neoantigen release is leveraged by PD-L1 blockade to stimulate a robust T-cell response. This specific combination is being studied in clinical trials led by West China Hospital of Sichuan University.
