Drug intelligence / Profile preview
Xacduro + meropenem is a co-administration regimen combining Xacduro (sulbactam-durlobactam), a novel beta-lactam/beta-lactamase inhibitor, with meropenem, an ultra-broad-spectrum carbapenem antibiotic. Developed by Innoviva (via its subsidiary Entasis Therapeutics) and Pfizer/Sumitomo Pharma respectively, this combination is primarily utilized for the treatment of severe, invasive infections caused by carbapenem-resistant *Acinetobacter baumannii-calcoaceticus* complex (CRAB), such as hospital-acquired and ventilator-associated bacterial pneumonia (HABP/VABP). Sulbactam acts as a beta-lactam antibiotic with intrinsic activity against *A. baumannii* by targeting penicillin-binding proteins (PBPs), specifically PBP1 and PBP3. Durlobactam is a diazabicyclooctane beta-lactamase inhibitor that potently inhibits Ambler Class A, C, and D serine beta-lactamases (including OXA-type carbapenemases), thereby restoring sulbactam's bactericidal activity. Meropenem, a carbapenem that inhibits cell wall synthesis by binding to PBPs, is co-administered to provide synergistic antibacterial activity and expand coverage against polymicrobial infections. This combination is recommended by the Infectious Diseases Society of America (IDSA) as a preferred therapeutic strategy for moderate-to-severe CRAB infections.
