Sulfadoxine-pyrimethamine + mefloquine (Fansimef) is a fixed-dose combination antimalarial medication that combines three active ingredients: mefloquine, sulfadoxine, and pyrimethamine. This combination works by targeting multiple stages of the malaria parasite's lifecycle. Sulfadoxine inhibits dihydropteroate synthase (DHPS) and pyrimethamine inhibits dihydrofolate reductase (DHFR), both enzymes in the folate biosynthesis pathway of Plasmodium species. Mefloquine adds another mechanism of action to enhance efficacy against resistant strains. The combination was developed to treat chloroquine-resistant Plasmodium falciparum malaria.
