BESIPTOL-NIKA is a generic, fixed-dose combination antibacterial drug product developed and manufactured by Nika Farm in Uzbekistan. It contains sulfamethoxazole, a sulfonamide antibiotic, and trimethoprim, a dihydrofolate reductase inhibitor, in a 5:1 ratio (400 mg / 80 mg, totaling 480 mg). This combination, commonly known as co-trimoxazole, exhibits synergistic bactericidal activity by sequentially blocking two consecutive steps in the bacterial biosynthesis of essential nucleic acids and proteins. Specifically, sulfamethoxazole competitively inhibits dihydropteroate synthase (DHPS) to prevent the synthesis of dihydrofolic acid, while trimethoprim reversibly inhibits dihydrofolate reductase (DHFR) to block the conversion of dihydrofolic acid to tetrahydrofolic acid. BESIPTOL-NIKA is indicated for the treatment of various bacterial infections, including urinary tract infections, acute otitis media in children, exacerbations of chronic bronchitis, nocardiosis, toxoplasmosis, and Pneumocystis jirovecii pneumonia (PCP).
