Sulfapyridine is a sulfonamide antibiotic and anti-inflammatory small molecule that was historically used to treat dermatitis herpetiformis (Duhring’s disease), benign mucous membrane pemphigoid, pyoderma gangrenosum, rheumatoid arthritis, ulcerative colitis, Crohn’s disease, and systemic lupus erythematosus[1][2][5]. Its primary mechanism of action is as a competitive inhibitor of the bacterial enzyme dihydropteroate synthase, which blocks folic acid synthesis in susceptible bacteria by interfering with the processing of para-aminobenzoic acid (PABA)[2][7]. Sulfapyridine is also a major metabolite of sulfasalazine. It has been discontinued for marketing in many regions due to safety concerns and the availability of better alternatives[1][3].
